a plan of care specifically for your needs, describing the exact
care to be provided. To assure an easy transition to home care,
our clinical service providers will coordinate your health care
with you, your family, and your personal physician prior to
beginning service.
Services include skilled nursing care, home health aide services,
medical social services, and physical, occupational, and speech
therapy services.
